https://www.kobotoolbox.org/ is one option. loomio/codema.in count poll is another option. Lime Survey or other such survey tools could be self hosted as well. We will need similar form for participants as well.

There should be some criterion applied to make sure the philosophy of mentors align with the philosophy of the camp. I don't think everyone who wants to be a mentor should automatically be a "mentor". It should be a carefully curated bunch. Can be everyone who volunteers, but it shouldn't be a casual decision, rather a deliberate decision.

How about we include the following questions?

• Would you consider yourself aligned with Free Software philosophy or Open Source development model? Options: Free Software philosophy, Open Source development model
• What was your motivation to start using Free Software?
• What inspired you to contribute to Free Software?
• What are your goals of being a mentor in this camp?

Sounds great.

It is also perhaps useful to add a question that is purely to make mentors think about the learners, like:

• In your opinion, what are the problems learners face in contributing to free software projects? How will you help them overcome these?

Just to make them empathetic and prepare themselves for the days ahead.

In my opinion the following questions are enough for mentors:

• Would you consider yourself aligned with Free Software philosophy or Open Source development model? Options: Free Software philosophy, Open Source development model
• What are your goals of being a mentor in this camp?

If people are being ready to mentor that means they will spend good amount of time on it. Above this if we are trying to educate them with our big set of questions then it may be annoying. We can just add two criteria to be a mentor - patient and respectful. Apart from that I don't see why we need to measure all the aspects of a person. Ready to be a mentor without expecting any particular personal gain (apart from their own learning curve) means that they are already empathetic.

I feel explicitly stating their commitment to Free Software philosophy is an important factor for mentors in the camp. Otherwise, we have chances to get Open source people volunteering which is not our aim. On another note, if one is patient enough to mentor, answering a couple of questions should not annoy them. I am not saying we should ask a long list of questions, but a couple of them to give a clear understanding about them will not hurt. In fact, the quality of mentors will directly affect the outcome of the camp.

I agree with @Srud that asking some details about mentors to know them better is fine, it is even essential, I think.

Yeah, we needn't be pessimistic. I may be carrying that feeling over from hackathons where mentorship is casual, ill-thought, unstructured, doesn't follow any pedagogy, and is rather limited in its role.

In fact, rather than make a barrier at entry, we can try to help whoever volunteers to be effective at mentorship through a set of resources which I'm tracking in #7
