add XMPP and IRC contact information

PS : both SVG images are kind of hacks. they was traced from PNG bitmaps and hence might not be as scalable as other true vector graphics

Looks like typo on irc.sv file name, did you mean irc.svg?

where exactly did you notice the typo @praveen ? I cannot seem to find it.

Also conversations.im link needs # replaced by %23 to work (in diasp.in also you might want)

@shine i tried opening the svg files in both firefox and inkscape and for some weird reason, they are rendered as blank images!
